PA SPREAD aims to build on the success of Pennsylvania’s Chronic Care Initiative, led by the PA Department of Health’s Executive Deputy Secretary, Michael Wolf, and Director of Chronic Care, Marcela Myers.

Over the past 4 years, the PA Chronic Care Initiative and similarly designed efforts in PA have helped nearly 200 primary care practices implement the Medical Home model and improve quality.
